Health Services
Bellwood School District 88 is fully committed to the health of our students. Our health services staff looks after each student’s physical and mental well-being while on our campus.
Administration of Medication
Certain health problems require that students take medication during school hours. Medication must be always kept in the health office.
- Medication must be in its original, properly labeled container. Prescription medication must have the pharmacy label, as this serves as the doctor’s order.
- A signed consent form giving specific directions for use must accompany medication from the parent.
With parent permission, students with severe allergies may carry and self-administer an inhaler or EpiPen that has the proper labeling. Please check with the school office to obtain and sign the "Permission to Carry" form. We encourage students to report to an adult or to the office after using the EpiPen.
Health Screenings and Records
We ask that parents maintain recommended health screenings for each of their children and keep school records updated with the following:
- Physical exams and updated immunization records for students entering preschool, kindergarten, and sixth grade
- Dental exams for students in kindergarten, second, and sixth grade
- We give hearing screenings every year to kindergarten, first, second, and third-grade grade students. We also screen all students receiving special education services.
- We administer vision screenings each year for all kindergarten, second, and eighth-grade students as well as those receiving Special Education services.
We will notify you by mail if your child needs additional immunizations before the start of a new school year. State law requires students to complete all immunizations before school starts. Students who cannot provide proof of required immunizations will not be allowed to attend school or ride the bus on the first day of classes. Please submit immunization updates to the school office.

The district carries student insurance, however, parents are responsible for any insurance claims for medical treatment of their children outside the scope of our insurance.
Student Athletes:
All student athletes must submit a physical exam and a signed concussion notice to participate in any school-sponsored sport teams.
